Read the complete details of the contest carefully

Step 3

Scroll down to the bottom of the page. Click the entry form link

Step 4

The application form will be displayed on the screen. Take a print out of it

Step 5

Send one hard copy of your entry and two copies of the entry form (one firmly attached and another loosely attached with a gem clip) to NASA.
